User Agreement for Freedom & Legacy Blueprint MD Coaching Program (12-Month Program)

 

IMPORTANT: READ CAREFULLY BEFORE ACCESSING OR USING ANY MATERIALS ASSOCIATED WITH THE FREEDOM & LEGACY BLUEPRINT MD COACHING PROGRAM.

 

BY ACCESSING OR USING THE PROGRAM(S), YOU REPRESENT, WARRANT, AND AGREE THAT:

  • YOU HAVE READ THIS AGREEMENT IN FULL;

  • YOU UNDERSTAND ITS TERMS;

  • YOU ACCEPT AND AGREE TO BE LEGALLY BOUND BY THIS AGREEMENT.

IF YOU DO NOT AGREE, UNDERSTAND, OR ACCEPT THIS AGREEMENT, DO NOT ACCESS OR USE ANY MATERIALS OR SERVICES ASSOCIATED WITH THE FREEDOM & LEGACY BLUEPRINT MD COACHING PROGRAM AND CONTACT US IMMEDIATELY AT rachel@blackswanteam.com 

Section 1: Membership and Program Fees

1.1 Programs. Freedom & Legacy Blueprint MD Coaching Program (“Company”) agrees to provide educational coaching services in the form of webinars, audio and/or visual presentations, PDF/Word documents, group coaching, and related materials (“Program(s)”) in exchange for the Program Fee.

1.2 Program Fees. YOU agree and understand that Program Fees are non-refundable under any circumstances. By enrolling, YOU commit to pay:

  • (A) $3,999 per month for four months, (If you choose the payment plan option, all payments are due in full regardless of your participation in the program), or
  • (B) a one-time payment of $15,000

1.3 No Refunds, No Chargebacks.

  • By enrolling, YOU acknowledge that all Program Fees are earned in full upon payment and strictly non-refundable.

  • YOU waive any right to initiate a chargeback, reversal, or dispute of payments made.

  • In the event of a credit card dispute, this Agreement will be presented as conclusive evidence of your acceptance of the no-refund, no-cancellation terms.

  • Failure to pay Program Fees in full does not release YOU from the obligation to pay in full.

1.4 Promotional Material. By participating, YOU grant Company the right to use testimonials, written statements, images, or recordings referencing your participation for marketing purposes.

Section 2: Disclaimers & Limitations

2.1 Educational Purposes Only.
The Program is provided solely for educational and informational purposes. It does not constitute legal, medical, financial, tax, or business advice. YOU are solely responsible for how you use or do not use the information provided.

2.2 No Physician-Patient Relationship.
While Dr. Elaine Stageberg, MD, MHA is a licensed physician in Minnesota, this Program does not create, and shall never create, a physician-patient relationship. No medical diagnosis, treatment, or individualized medical advice will be provided. If YOU have medical or mental health concerns, YOU agree to seek independent care from a licensed healthcare provider outside of this Program.

2.3 No Guarantees of Results.

  • The Program makes no guarantees or warranties of specific outcomes, earnings, health improvements, business success, or personal results.

  • Examples, testimonials, and case studies are illustrative only and are not promises of future performance.

  • YOU may experience losses or no results whatsoever.

2.4 Limitation of Liability.
To the fullest extent allowed by law, Company, its owners, officers, employees, and affiliates shall not be liable for any damages of any kind, including direct, indirect, incidental, consequential, or special damages, arising out of or relating to the Program.

2.5 Assumption of Risk.
By enrolling, YOU expressly assume all risks related to decisions you make regarding health, investing, business, relationships, or any other aspect of your life. YOU accept full and sole responsibility for your outcomes.

Section 3: Confidentiality & Intellectual Property

3.1 Confidentiality. Access is personal to YOU only. Sharing of login credentials, materials, or recordings is strictly prohibited.

3.2 Intellectual Property. All Program content is owned exclusively by Company. YOU receive a limited, revocable license for personal use only. No rights are granted to copy, share, or distribute materials.

Section 4: Miscellaneous

4.1 Non-transferability. Enrollment is personal to YOU and cannot be transferred.

4.2 Indemnification. YOU agree to indemnify, defend, and hold harmless Company, its officers, employees, and affiliates from any claims, liabilities, damages, costs, or expenses (including attorney’s fees) arising out of your participation in the Program.

4.3 Entire Agreement. This Agreement constitutes the full understanding between the parties and supersedes all prior agreements.

4.4 Amendment. Company reserves the right to amend this Agreement by posting an updated version online.

4.5 Governing Law. This Agreement shall be governed by the laws of the State of Minnesota.

4.6 Legal Expenses. In the event of legal action to enforce this Agreement, the prevailing party shall be entitled to recover attorney’s fees and costs.
